LATAM | Senior AI Engineer

Location: Latin America (ideally São Paulo, Bogotá, Mexico City, Buenos Aires or Chile)

Employment Type: Full-time

Salary: Up to $130,000 USD/year

About the role

We're working with a healthcare AI company building AI-powered solutions that integrate directly into clinical workflows across Latin America.

They are looking for a Senior AI Engineer to take their production AI agents to the next level through model fine-tuning, clinical knowledge systems, and robust evaluation frameworks.

This is a highly technical, hands-on Individual Contributor role focused on building and shipping AI systems into production.

About the company

The company was born at Stanford and is built by doctors, for doctors. Its mission is to transform healthcare in Latin America and beyond. The platform introduces AI Healthcare Employees that work invisibly inside clinical workflows. They transcribe consultations in real time, generate structured medical records, offer evidence-based decision support, and integrate with any existing health system, all without disrupting the physician's flow.

It's a small team working on hard problems, building systems that doctors across Latin America use every day.

What you'll do

  • Design clinical ontologies and structured knowledge representations for agents, so decision support becomes deterministic and auditable
  • Fine-tune and distill small LLMs to specialize and accelerate the agent stack, from data curation through deployment
  • Decide when a problem needs better context, model training, or deterministic code, and justify the call on quality, latency, and cost
  • Apply context engineering as measured engineering (retrieval, compression, memory, structure), not prompt tweaking
  • Build router models and task decomposition that make the easy 80% cheap and fast and the hard 20% correct
  • Push evaluation beyond the standard playbook: adversarial and counterfactual evals, calibrated LLM-as-judge, failure taxonomies, and uncertainty estimation
  • Own these systems in production (monitoring, cost, failure modes) and set the bar for how agents are built and evaluated

What we're looking for

  • Experience building and shipping AI agents / LLM-based systems in production
  • Hands-on experience training, fine-tuning, or distilling models end to end, where the result mattered
  • Experience designing evaluation systems that caught what standard metrics didn't
  • Clear judgment on when to use context versus training
  • Experience with RAG, vector stores, knowledge graphs, context engineering, and grounding agents against large-scale data
  • Ability to look at a clinical rule and decide what belongs in code, in an ontology, or in the model, and defend that split
  • Strong Python skills, plus the ability to read research and tell which results hold up in production
  • Experience maintaining AI systems in production
  • AI-native: uses AI coding tools daily
  • Strong individual contributor mindset
  • Fluent English
  • Based in Latin America
